    Antibiotic Resistance of Vibrio cholerae Isolates from Kashan, Iran

    Aims: Cholera is an acute diarrheal disease that can lead to severe dehydration and death. Antibiotic resistance is a big challenge in infective disease like Cholera. The present study aimed to understand the characteristics and trends of antibiotic resistance of V. cholerae isolations in and around Kashan, Iran. Instrument & Methods: In this descriptive cross-sectional study, samples were gathered using census method from 1998 to 2013 in Kashan, Iran. 1132 fecal samples of patients with acute diarrhea and 237 samples of suspected water samples were taken. The serotypes and biotypes were determined by an enzymatic method. Antibiotic susceptibility test was performed by using Disk Diffusion Method. Data were analyzed using SPSS 23 software. Fisher-exact and Chi-square tests were used to compare the statistical parameters. Findings: 96 fecal samples (8.5) and 18 water samples (7.6) were positive for Vibrio cholerae. Non-agglutinating (Nag) isolates (75.4) were more common than serotype Inaba (13.2) and Ogawa (11.4). Nag serotypes were mostly resistant to cefixime (44) and ampicillin (33). In contaminated water samples also the most frequent cases were Nag serotype (50). Nag serotype showed 22.2 of resistance to ampicillin and nitrofurantoin. Conclusion: Vibrio cholerae isolates in Kashan, Iran, are highly resistant to antibiotics, especially Nag serotypes

    A Time Series Model for Assessing the Trend and Forecasting the Road Traffic Accident Mortality

    Background: Road traffic accident (RTA) is one of the main causes of trauma and known as a growing public health concern worldwide, especially in developing countries. Assessing the trend of fatalities in the past years and forecasting it enables us to make the appropriate planning for prevention and control. Objectives: This study aimed to assess the trend of RTAs and forecast it in the next years by using time series modeling. Materials and Methods: In this historical analytical study, the RTA mortalities in Zanjan Province, Iran, were evaluated during 2007 - 2013. The time series analyses including Box-Jenkins models were used to assess the trend of accident fatalities in previous years and forecast it for the next 4 years. Results: The mean age of the victims was 37.22 years (SD = 20.01). From a total of 2571 deaths, 77.5% (n = 1992) were males and 22.5% (n = 579) were females. The study models showed a descending trend of fatalities in the study years. The SARIMA (1, 1, 3) (0, 1, 0) 12 model was recognized as a best fit model in forecasting the trend of fatalities. Forecasting model also showed a descending trend of traffic accident mortalities in the next 4 years. Conclusions: There was a decreasing trend in the study and the future years. It seems that implementation of some interventions in the recent decade has had a positive effect on the decline of RTA fatalities. Nevertheless, there is still a need to pay more attention in order to prevent the occurrence and the mortalities related to traffic accidents

    Factors affecting the outcome of community-acquired pneumonia among the patients hospitalized in Beheshti hospital (Kashan-Iran)

    Background: Community-acquired pneumonia (CAP) is a common infectious disease with high morbidity and mortality. The goal of this study was to evaluate the factors affecting the outcome of pneumonia among the Beheshti hospital patients. Materials and Methods: This cohort study was done on pneumonia patients (n=140) in Kashan Beheshti hospital during 2014-2015. A questionnaire consisting the demographic, clinical and paraclinical findings and outcomes was filled-out. Results: Eighty three (59.3) out of 140 patients were male and 57(40.7) women. The majority of cases were ≥50 years old (mean age 60.02±1.70) .There was a history of diabetes in 54(38.6) .The most common signs and symptoms were coughing and the lung rales. The ninty-one and 9 of the cases were improved and complicated condition, respectively. The complication were: pleural effusion (77), empiyema (15) and abscess (8).There was positive CRP (100); increased ESR (82) and leukocytosis (80). While, there was no statistical association between the sex, age and clinical symptoms with the disease complication and outcome, there was a significant correlation between the first BS, HbA1C, CRP, duration of hospitalization, radiographic pattern and diabetes with disease outcome. Conclusion: Considering the association between the diabetes in one side and some factors (outcome of pneumonia, duration of hospitalization, history of pneumonia, times of admission, BS at admission, HbA1c, bilateral involvement, leukocytosis, increased ESR, CRP and CURB 65, the diabetes should be considered as an important factor affecting the pneumonia outcome. The managed control of diabetes can improve the pneumonia outcome

    Magnesium Supplementation and the Effects on Wound Healing and Metabolic Status in Patients with Diabetic Foot Ulcer: a Randomized, Double-Blind, Placebo-Controlled Trial

    Hypomagnesemia is associated with the development of neuropathy and abnormal platelet activity, both of which are risk factors for diabetic foot ulcer (DFU). This study was carried out to evaluate the effects of magnesium administration on wound healing and metabolic status in subjects with DFU. This randomized, double-blind, placebo-controlled trial was performed among 70 subjects with grade 3 DFU. Subjects were randomly divided into two groups (35 subjects each group) to receive either 250 mg magnesium oxide supplements or placebo daily for 12 weeks. Pre- and post-intervention wound depth and appearance were scored in accordance with the “Wagner-Meggitt’s” wound assessment tool. Fasting blood samples were taken at baseline and after the 12-week intervention to assess related markers. After the 12-week treatment, compared with the placebo, magnesium supplementation resulted in a significant increase in serum magnesium (+0.3 ± 0.3 vs. −0.1 ± 0.2 mg/dL, P < 0.001) and significant reductions in ulcer length (−1.8 ± 2.0 vs. −0.9 ± 1.1 cm, P = 0.01), width (−1.6 ± 2.0 vs. −0.8 ± 0.9 cm, P = 0.02), and depth (−0.8 ± 0.8 vs. −0.3 ± 0.5 cm, P = 0.003). In addition, significant reductions in fasting plasma glucose (−45.4 ± 82.6 vs. −10.6 ± 53.7 mg/dL, P = 0.04), serum insulin values (−2.4 ± 5.6 vs. +1.5 ± 9.6 ÎŒIU/mL, P = 0.04), and HbA1c (−0.7 ± 1.5 vs. −0.1 ± 0.4%, P = 0.03) and a significant rise in the quantitative insulin sensitivity check index (+0.01 ± 0.01 vs. −0.004 ± 0.02, P = 0.01) were seen following supplementation of magnesium compared with the placebo. Additionally, compared with the placebo, taking magnesium resulted in significant decrease in serum high-sensitivity C-reactive protein (hs-CRP) (−19.6 ± 32.5 vs. −4.8 ± 11.2 mg/L, P = 0.01) and significant increase in plasma total antioxidant capacity (TAC) concentrations (+6.4 ± 65.2 vs. −129.9 ± 208.3 mmol/L, P < 0.001). Overall, magnesium supplementation for 12 weeks among subjects with DFU had beneficial effects on parameters of ulcer size, glucose metabolism, serum hs-CRP, and plasma TAC levels

    A New Optimization Strategy to Improve Design of Hydrogen Network Based Formulation of Hydrogen Consumers

    This paper describes a shortcut model for formulating hydrogen consumers in hydrogen network based on inlet/outlet flow rate and inlet/outlet hydrogen purity. The formulation procedure is obtained using nonlinear regression of industrial data and represents the relationship between the flow rate and purity of outlet and inlet streams. The proposed model can estimate outlet flow rate and purity of hydrogen by changing inlet flow rate and purity of hydrogen.     Supplemental irrigation management of rainfed grapevines under drought conditions using the CropSyst model

    Aim of study: To determine how much water should be used and when it should be applied in rain-fed grapevine using a cropping system simulation model (CropSyst), and also the economic analysis of supplemental irrigation for rainfed grapevine.Area of study: This study was conducted at the School of Agriculture, Shiraz University, Shiraz, Iran, in 2012, 2013 and 2014.Material and methods: The CropSyst model was calibrated to predict the rainfed yields of ‘Askari’ and ‘Yaghooti’ grapevines in different climates using four amounts of SI: 250 L (I1), 500 L (I2), 1000 L (I3) and 0 (I4), five SI times: single in March (T1), single in April (T2), single in March + single in April (T3), single in May (T4) and single in June (T5).Main results: Treatment T3 increased the average simulated yield of ‘Askari’ by 15% to 40% at regions with P/ETo&gt;0.6, 17% to 61% at 0.2&lt;P/ETO&lt;0.6, and 26% to 61% at P/ETO&lt;0.2, while in ‘Yaghooti’ it increased about 2% to 41% at regions with P/ETo&gt;0.6, 4% to 36% at 0.2&lt;P/ETO&lt;0.6 and 2% to 26% at P/ETO&lt;0.2. By increasing the water price by 30% and 50%, net benefits for the ‘Askari’ decreased by about 31% and 54%, while 6% and 18%, for ‘Yaghooti’ respectively.Research highlights: The CropSyst model can successfully predict soil water content and grapevine yields. Application of SI in May increased significantly the grapevine yield as compared to other SI times

    Mycobiota and aflatoxin B1 contamination of rainbow trout (Oncorhinchus mykiss) feed with emphasis to Aspergillus section Flavi

    In the present study, mycobiota and natural occurrence of aflatoxin B1 (AFB1) in pellet feed and feed ingredients used in a feed manufacturing plant for rainbow trout nutrition was investigated. The samples were cultured on the standard isolation media for 2 weeks at 28 ÂșC. Identification of fungal isolates was implemented based on the macro- and microscopic morphological criteria. AFB1 was detected using high performance liquid chromatography (HPLC). Based on the results obtained, a total of 109 fungal isolates were identified of which Aspergillus was the prominent genus (57.0%), followed by Penicillium (12.84%), Absidia (11.01%) and Pseudallscheria (10.10%). The most frequent Aspergillus species was A. flavus (60.66%) isolated from all feed ingredients as well as pellet feed. Among 37 A. flavus isolates, 19 (51.35%) were able to produce AFB1 on YES broth in the range of 10.2 to 612.8 ”g/g fungal dry weight. HPLC analysis of trout feed showed that pellet feed and all feed ingredients tested except gluten were contaminated with different levels of AFB1 in the range of 1.83 to 67.35 ”g/kg. Unacceptable levels of AFB1 were reported for feed including soybean, fish meal and wheat. These results indicate the importance of AF contamination of trout feed in amounts higher than the acceptable level as a risk factor for fish farming production

    Magnesium-based nanocomposites: A review from mechanical, creep and fatigue properties

    The addition of nanoscale additions to magnesium (Mg) based alloys can boost mechanical characteristics without noticeably decreasing ductility. Since Mg is the lightest structural material, the Mg-based nanocomposites (NCs) with improved mechanical properties are appealing materials for lightweight structural applications. In contrast to conventional Mg-based composites, the incorporation of nano-sized reinforcing particles noticeably boosts the strength of Mg-based nanocomposites without significantly reducing the formability. The present article reviews Mg-based metal matrix nanocomposites (MMNCs) with metallic and ceramic additions, fabricated via both solid-based (sintering and powder metallurgy) and liquid-based (disintegrated melt deposition) technologies. It also reviews strengthening models and mechanisms that have been proposed to explain the improved mechanical characteristics of Mg-based alloys and nanocomposites. Further, synergistic strengthening mechanisms in Mg matrix nanocomposites and the dominant equations for quantitatively predicting mechanical properties are provided. Furthermore, this study offers an overview of the creep and fatigue behavior of Mg-based alloys and nanocomposites using both traditional (uniaxial) and depth-sensing indentation techniques. The potential applications of magnesium-based alloys and nanocomposites are also surveyed
